Si la quasi-totalit\u00e9 des neurones du striatum sont GABAergiques, seulement1% sont des interneurones cholinergiques. Ces interneurones g\u00e9ants g\u00e9n\u00e8rent une r\u00e9ponse caract\u00e9ristique \u00e0 un stimulus sensoriel associ\u00e9 \u00e0 une r\u00e9compense, une pause dans leur d\u00e9charge, qui disparait&nbsp;dans le striatum priv\u00e9 de ses aff\u00e9rences dopaminergiques.<\/p>\n\n\n\n<p>Un troisi\u00e8me neurotransmetteur,l\u2019acide g-aminobutyrique (GABA), s\u2019est invit\u00e9 dans la pathologie lorsque nous avons d\u00e9couvert qu\u2019environ 50% des interneurones cholinergiques du striatum sont aussi GABAergiques, en nous basant sur des marqueurs (Lhx6), de la PCR intracellulaire (gad2) et des enregistrements de paires de neurones. Faiblement connect\u00e9s entre eux chez les animaux t\u00e9moins,ces interneurones mixtes lib\u00e8rent ac\u00e9tylcholine et GABA, agissant sur des r\u00e9cepteurs nicotiniques et GABA<sub>A<\/sub>, pour g\u00e9n\u00e9rer des courants antagonistes, entrant et sortant, respectivement. Dans le striatum d\u00e9pl\u00e9t\u00e9 endopamine, les connexions entre neurones mixtes augmentent et le courant GABA<sub>A<\/sub> devient entrant, tout comme le courant cholinergique, abolissant ainsi la\u00ab&nbsp;pause-r\u00e9ponse&nbsp;\u00bb \u00e9voqu\u00e9e par la stimulation des aff\u00e9rences corticales. Baisser les taux de (Cl<sup>&#8211;<\/sup>)<sub>i<\/sub> avec la bum\u00e9tanide, un bloquant de l\u2019importateur des ions chlorures, NKCC1, r\u00e9tablit la polarit\u00e9 du courant GABA<sub>A<\/sub> et la pause-r\u00e9ponse in vitro. In vivo, le traitement chronique \u00e0 la bum\u00e9tanide des souris dont un des striatum est d\u00e9pl\u00e9t\u00e9 en dopamine, am\u00e9liore leur motricit\u00e9. <\/p>\n\n\n\n<p>En parall\u00e8le, nous avons conduit un essai clinique ouvert sur 4 patients avec des effets prometteurs de la bum\u00e9tanide. Nous allons bient\u00f4t effectuer un essai en double aveugle sur 40patients, centr\u00e9 sur les probl\u00e8mes de chute et de \u00ab&nbsp;freezing&nbsp;\u00bb des parkinsoniens. Collectivement, ces travaux illustrent l\u2019importance du m\u00e9tabolisme neuronal des ions chlorures, perturb\u00e9 dans de nombreuses pathologies neurologiques.<\/p>\n\n\n\n<p><strong>Three neurotransmitters and Parkinson\ndisease<\/strong><\/p>\n\n\n\n<p>The progressive loss of dopaminergic innervation of the basal ganglia, in particular of the dorsal striatum (putamen) is responsible for the motor signs of Parkinson disease such as bradykinesia, rigidity and loss of postural reflexes. In the striatum, the vast majority of neurons are GABAergic, with cholinergic interneurons representing only 1% of the total population. These giant interneurons gradually develop a typical conditioned pause response during acquisition of a sensorimotor association. The proportion of these responsive interneurons is dramatically decreased in the dopamine-depleted striatum.<\/p>\n\n\n\n<p>We recently identified a third player in the disease, the gamma-aminobutyric acid (GABA). On the basis of &nbsp;intracellular markers labeling (Lhx6), RTqPCR (gad2) and pair recordings, we showed that around 50% of cholinergic interneurons also synthesize and release GABA. Loosely interconnected in control striatum, these mixed interneurons generate inward nicotinic and outward GABA<sub>A<\/sub> currents. In the dopamine-depleted striatum, interconnections dramatically increase and the GABA<sub>A&nbsp;<\/sub>current becomes inward like the nicotinic one, leading to disappearance of the pause (suppression of firing) in response to cortical afferents stimulation. Application in slices of the blocker of the chloride importer NKCC1, bumetanide, to lower intracellular chloride concentration, reestablishes GABA<sub>A<\/sub>current polarity and the pause response. Chronic bumetanide treatment ameliorates the motricity of mice with dopamine-depletion in one striatum.<\/p>\n\n\n\n<p>In parallel, we conducted a promising open study with 4 patients which will be followed by a double-blind essay with 40 patients, targeting problems of posture, falls and freezing, in particular.
